If you are planning on attending a youth ministry event, please be aware that we require there to be an up-to-date release and liability form on hand with an up-to-date copy of each student's insurance card.  Unless otherwise notified, the release and liability form is good for all youth events, activities and programs for one year from the date of signing.  Also, please remember to have both sutdent and parent(s)/Guardian(s) sign the appropriate sections.
